Pietro, age 7, motor disability: improving independence in communication
Editorial data: Portale SIVA Fondazione Don Carlo Gnocchi Onlus
Abstract: Pietro is 6 years old and he is affected by dystonic quadriplegia and impaired upper limb functionality; he can articulate just a few words, but shows a good level of comprehension. This paper describes the way towards autonomy that the child, his family and the caregivers experienced together. At the beginning, parting from the mother was a great effort for the children, who expressed his discomfort rejecting unknown people and environments. Later, the emotional instability gradually decreased; Pietro became conscious of his abilities, he found fulfilment in them and his interests increased. Therefore it was possible to start a project aiming to the evaluation of assistive devices allowing Pietro to interact with the environment and actively take part in educational activities. An orthosis, keeping the thumb and the wrist aligned, was prescribed for the less impaired arm. In this way Pietro, who was already able to use a device with automatic scansion, could use a communication aid whose vocal output was activated by the pressure of a button. Besides, a multimedia "open" software for the creation of customized educational unities was chosen. Finally, Pietro was given a postural wheelchair allowing the carriage of the communicator and the use of the computer without tiring transfers from the push-chair to the wheelchair.

Andrea, 6 years old. Autonomy in daily life for a person with cerebral palsy
Editorial data: Portale SIVA FondazioneDon Carlo Gnocchi
Abstract: Andrea is a child aged 6, suffering from cerebral palsy and his mother is his main caregiver. In 2007 he started attending the kindergarten, although not constantly, because of the considerable difficulties of displacement, with his mother carrying him in her arms. At home Andrea used a seating system far too small (prescribed 2 years before), while the kindergarten used the same stroller used outdoors because the school did not have any seating system. Our work was therefore focused on finding solutions aimed at improving the situation, in order to lighten the burden of care of the mother and offering to Andrea a better posture, thus opening a new outlook. The solutions adopted at the end of the evaluation focused on the posture aids, autonomy in food, reduction of the burden of care and prevention of stabilized leg deformities.

Abstract: Roberta is 38 years old and she was affected by a meningoencephalomyelitis 7 years ago. She moves very slowly and she finds all her activities very tiring. She lives in a scarcely accessible flat on the third floor without lift, and she rarely goes out. In the years her conditions got worse: now she gets rapidly tired when she walks, climbs the stairs, washes heself and does the houseworks, so she has to reduce her free time activities. She would like to discover aids and solutions that help her going round the city and to the supermarket idependently, washing herself in the bathroom in an less tiring way and using the gas cooker and the sink. She has a manual wheelchair which she can't use in her small flat. She has a driving licence and a special car but she can't use it very much, because she doesn't know the roads. Analyzing her demands and considering the assistive devices she has got, a solution for her problems was found. Fundamentals of seating clinic
Editorial data: Dispense Corso di Alta Formazione "Ausili per l'Autonomia e la Partecipazione" - Associazione La Nostra Famiglia IRCCS E.Medea
Abstract: (text automatically translated from Italian) The lesson examines the ideal sitting posture in the "normal" person (no neurological impairments which are essentially and / or orthopedic surgery) from the point of view of kinematics (arrangement of the different segments in space - joint angles), the kinetics (forces and moments involved in the maintenance of posture, stress and strain at the level of biological structures) and neurophysiological control mechanisms. The analysis is directed to the individual body segments, starting from the basin, and buoyancy to the influence of each of these on the entire system. Is then taken into account in the sitting posture in individuals with disabilities analyzing the problems most frequently encountered in recruitment and maintenance of this posture and proposing several possible solutions designed to guarantee maximum safety, maximum comfort and maximum functionality, with particular reference systems posture. Of the latter explains the main features and mechanisms of action.
