Abstract:
Individual assessment for assistive technology requires a detailed analysis of the user's needs, the ability to guide the user to establish priorities and define objectives, and the ability to formulate recommendations for the most appropriate solutions. In good practice, the outcome of an individual assessment should be writted down in a good assessment report that synthesises all the carried-out analyses and trials, describes the recommended solutions and enclosed all needed documentary evidence. The enclosed template - which is freely downloadable and can be used by anybody interested - is the result of many years of research and experimental trials witin the Current Research programme of the Fondazione Don Gnocchi Clinical Research Institute (under the Biomedical Research Programme of the Italian Ministry of Health). It can track the whole assistive technology service delivery process from the initial assessment (assistive technology recommandation) to the verification of the assistive solutions provided and the subsequent follow-up. TECHNICAL NOTES : the file is a fillable PDF; it was developed by means of a specialized software (Adobe LiveCycle Designer) but can be opened and filled-in by means of any free PDF reader (such as Acrobat Reader DC).
