Abstract: Francesco is aged 50 and in 2005 had a cerebral hemorrhage, that damaged both the motor abilities, resulting in hemiplegia, with aphasia and cognitive abilities, with aphasia, attention and body image disorders. This project aims to bring attention to the path that was made by Francesco, his family and all professionals who followed him. The hardest part of this work was the family’s acceptation of an unexpected situation, leading to a sudden new planning of life in general. Thanks to a close collaboration with the family and especially a strong motivation by Francesco, the solutions adopted granted a better quality of life for him. The main objectives of this project were the provision of aids and then the return home of Francesco, more and more the protagonist of his own choices.

Abstract: This paper concerns a case of a 18 years old boy, diagnosed with severe dystrophic epidermolysis, requiring a prescription for a new power wheelchair. Tato attends high school, supported by a teacher for assistance. Tato benefits of home care (nursing), and is followed by various specialists, but there is no meeting point between these various health and social figures. During counselling sessions Tato highlighted the need to have a more active social life outside the family and greater autonomy from the teacher at school. The project, will deal not only with the choice of the wheelchair and its customization, but also with manipulation aspect, the independent use of computer and the evaluation of devices already in use both at home and at school. The aim is also to create a multidisciplinary team of professionals, working in synergy.
